Description

To work as part of a small geographical team of Home Care assistants, under the direction of the Care Manager. To provide quality personal assistance with care tasks to meet the needs of individual clients in order for them to continue living in the home of their choice with minimum reliance or assistance from others.

Intervention focusing on the redevelopment of individuals self-care skills together with their confidence to utilise them.

To provide high standard services as specified in agreed care plans and to contribute to the continual reassessment of individual needs as part of a team approach.

Key Accountabilities:

To be responsible for the delivery of personal and practical care assistance in accordance with the care plans of a number of individual clients; this may include:

  • Assistance with physical tasks e.g, washing, dressing, feeding, toileting, medication, incontinence, exercise routines (as part of re-ablement programme only).
  • Limited domestic duties e.g, laundry, cooking, cleaning (as part of an agreed care plan).
  • Limited social tasks to help individuals to maintain community and family involvement.
  • Financial matters – assistance with shopping and benefit collection.
  • To actively promote, encourage and support clients to maximize independence including participation in any re-habilitation programme.
  • To work in accordance with practices, procedures, and policies laid down by Beavers (Worcester) Ltd and in particular, the Carer’s Handbook.
  • To promote equality for all individuals.
  • To liaise with other professionals as part of a multi-disciplinary team.
  • To maintain confidentiality of information in line with our Code Of Practice.
  • To administer and record medication as prescribed to clients in accordance with Beavers (Worcester) Ltd policy on medication.
  • To maintain log books to ensure that information is available for those people who are entitled and need to know.
  • To accept and seek consultation and supervision from the Care Manager or any manager who holds overall responsibility for co-ordinating and monitoring the Care Plans of which the Home Care Assistant is a part.
  • To observe, monitor and feedback client/carer situation changes (environmental and individual) and to report any concerns.
  • To attend training sessions as requested by the Care Manager.
  • To comply with responsibilities under the Health & Safety at Work Act, including Moving & Handling, Food Hygiene and First Aid.

Other Duties:

  • To undertake such other duties, training and/or hours of work as may be reasonably required and which are consistent with the general level of responsibility of this job.
  • To undertake Health & Safety duties commensurate with the post and/or as detailed in Beavers (Worcester) Ltd Health & Safety Policy
